I am an out-of-network provider for all other insurance carriers. Please check with your carrier for your out-of-network benefits, including any deductible that must be met. I will provide you with paperwork for reimbursement from your insurance company if you are seeking out-of-network benefits.  Please contact me for further information about my fees.
